Centre Aid Form & Verification

The Centre Aid Form collects additional financial and family information not requested on the FAFSA. Every student applying for need-based aid is required to complete this form each year and submit to Centre’s Financial Aid Office.

  1. Using username and password provided to you by the Admission Office, log into CentreNet
  2. Go to Applied Students tab
  3. Click on Centre Aid Form

Verification

One of every three students who files the FAFSA is selected for review in a process called verification. A comment will appear on the front page of your Student Aid Report (SAR) if you have been selected for verification.

Aid applicants who are selected for verification must provide additional documentation to Centre before they can receive a financial aid award. Specifically, students selected for verification must either use the IRS Data Retrieval to complete their FAFSA or mail us their 2021 IRS Tax Return Transcript. The Verification Worksheet must be completed and signed by the student and at least one parent (2 pages). This form should be returned to Centre’s Financial Aid Office when you have completed it. Both the parent and independent (on the FAFSA) students are required to submit proof of non-filing from the IRS if a 2021 federal tax return was not filed. Additional Verification Instructions

For Verification purposes, you can choose either Data Retrieval, Tax Return Transcript, or a signed copy of your most recent federal tax return including all schedules and W-2’s.

Since Oct. 1, 2017, Federal Student Aid has added security and privacy protections to the FAFSA tool. If you use this tool, tax return information will be encrypted and will not be displayed on the web page or on your FASFA. Instead, you’ll see “Transferred from the IRS,” in the appropriate fields.

IRS Data Retrieval Tool Instructions (Preferred)

  1. Go to studentaid.gov
  2. Click on Login under Returning User  
  3. Use FSA ID as requested
  4. Select Make Correction to FAFSA
  5. Go to Parent and/or Student Financials tab
  6. Answer questions to see if you are eligible to use the IRS Data Retrieval Tool
  7. Enter Parent FSA ID for Parent info and/or Student FSA ID for Student info and click on Link to IRS
  8. The next screen will inform you that you are leaving FAFSA on the Web. Click OK to continue
  9. The IRS.gov screen will appear next. Enter information requested and click on Submit
  10. Once the IRS has validated your identification, you will be provided a list of all possible fields from a tax return that could transfer back into the FAFSA form. (Actual data is encrypted and will not be viewable)
  11. Click on box under “Transfer My Tax Information into the FAFSA.” Then click on the Transfer Now button
  12. Follow instructions to return to FAFSA on the Web to complete the FAFSA
Remember to electronically sign and submit the FAFSA again! 

IRS Tax Return Transcript Request Online Instructions

Tax filers can request a transcript PDF, free of charge, by ordering online.

  1. Go to www.irs.gov
  2. Under the Tools section, click “Get Your Tax Record”
  3. Click “Get Transcript Online” or  Click “Get Transcript by Mail”
  4. Create an account
  5. Make sure to request a 2021 Tax Return Transcript PDF and send to address below

Verification of Non-Filing Letter Instructions

If you've never filed a federal tax return, you must submit a 4506-T Request for Transcript of Tax Return form.  

  1. Go to www.irs.gov
  2. Click on Get Your Tax Record
  3. Click on transcript types
  4. Click on Form 4506-T  
  5. Download, complete, and return to the IRS. (Select #7, Verification of Non-filing)
  6. Once you receive the letter, send to address below
